2. Preventative Dentistry

Golan Family Dentistry recommends bi-annual cleanings and check-ups for everyone to ensure good oral health. To prevent decay and cavities, patients are tested for high levels of oral bacteria using the CAMBRA method. Patients considered high risk for decay and periodontal disease are treated with preventativmeasures such as at home rinses and cleaning instruments.

Testing

Testing meters can evaluate your mouth chemistry and determine its potential for causing tooth decay.

Why wait for disease to start with early white spots or cavities when today it is possible to test for the acid-producing bacteria by taking a sample of the biofilm and testing it for acid producing bacteria? For example, if an acid producing biofilm is detected early, you can avoid passing these germs to others as parents and caregivers do unintentionally to infants. Or why not try to eradicate these bacteria by changing the conditions (pH balance) of the mouth? Testing meters can now give estimates of acid producing bacteria in 15 seconds. A reading from 0 – 1500 correlates with low risk and a reading of 1501-9999 correlates with higher risk. This simple and inexpensive test can also be repeated after treatment and is a great motivational tool for both patient and dental professional alike as everyone likes to know if change is working!

Diagnosis And Prognosis

A diagnosis can now be made based on your individual risk from the facts collected on your caries risk assessment form. This is important for three basic reasons: firstly, it is based on identifiable evidence; secondly, the risk can be modified based on recommendations and actions; and thirdly, repeating the procedure can objectively measure change to reduce your risk and improve your health. This affects your prognosis or the ability to predict the probable course and positive outcome from taking action.

Prescribe

In addition to the traditional things you've always been told to do like brush, floss, pay attention to your diet, and see your dentist regularly, treatment and preventive strategies are now based on your individual risk rather than treating everybody with the same approach.

If your risk is high, your dental professional may prescribe a program to reduce it. Some of the newer and more specific agents include:

  • Rinses containing a safe dilution of sodium hypochlorite solution that can kill bacteria and raise pH (lower acidity) in those individuals found to have high acid producing bacteria levels. Other products are available that are designed to balance an acidic mouth and keep the pH neutral and encourage more normal bacteria.
  • Rinses containing chlorhexidine, an antibacterial agent that has been used for eons to reduce bacteria, to disrupt their influence, and to lower the counts of those causing disease.
  • Fluoride containing rinses to strengthen enamel surfaces making them more resistant to decay while encouraging re-mineralization.
  • Xylitol, a natural sugar alcohol used for years as a sweetener and alternative to sucrose (table sugar), is known to disrupt the ability of acid-producing bacteria to thrive and attach to teeth. Xylitol is available in a rinse, spray, chewing gum, as well a breath-mint.

Once your risk has been modified and reduced, the level of prevention and treatment can be matched to it. For example, you can move from a treatment strategy to more of a preventive one.

Partnership

Based on known science, a CAMBRA dental professional can quickly and easily assess your risk and recommend appropriate measures that you should try at home as part of your daily oral/dental health regimen. The old “drill it and fill it,” “one-size fits all” approach to the treatment of tooth decay is no longer appropriate. It does not change the conditions that lead to the disease in the first place nor does it change your risk of infection.

Finally, it's about partnership. Your relationship with your dental office team is based on both a scientific approach as well as caring health professionals who have your best interest at heart. Working together, they will ensure that dental decay is a disease of your past so that you can enjoy a lifetime of good oral and dental health.

3. Gum treatment

Golan Family Dentistry has wonderful success using the laser to treat gum disease in a non-surgical and gentle manner. After laser therapy, the hygientist cleans the teeth and root surfaces to improve healing. Three-month follow-up is recommended.

Extractions, biopsies and cosmetic gum procedures are also performed with the laser. The laser minimizes post-operative complications such as swelling, bleeding and pain. That means a more pleasant experience during and after the procedure.

5. Root Canals

Root canals are less painful at Golan Family Dentistry than most other dental offices. The laser cleans and disinfects the root canal system gently, significantly improve the quality of the root canal and increase the success of the restored tooth.

Grinding can cause tooth loss, tooth and restoration failure and breakage, pain in the head and neck, and tension headaches Drs. Golan treat grinding with traditional methods but add computer software and sensor technology to analyze and adjust your bite more accurately. Dental Health Mouth Guards

Mouth guards are coverings worn over teeth, and often used to protect teeth from injury from teeth grinding and during sports.

There are three types of mouth guards:

  1. Stock mouth protectors are preformed and come ready to wear. They are inexpensive and can be bought at most sporting good stores and department stores. However, little can be done to adjust their fit, they are bulky, make breathing and talking difficult, and they provide little or no protection. Dentists do not recommend their use.

  2. Boil and bite mouth protectors also can be bought at many sporting goods stores and may offer a better fit than stock mouth protectors. The "boil and bite" mouth guard is made from thermoplastic material. It is placed in hot water to soften, then placed in the mouth and shaped around the teeth using finger and tongue pressure.

  3. Custom-fitted mouth protectors are individually designed and made in a dental office or a professional laboratory based on your dentist's instructions. First, your dentist will make an impression of your teeth and a mouth guard is then molded over the model using a special material. Due to the use of the special material and because of the extra time and work involved, this custom-made mouth guard is more expensive than the other types, but it provides the most comfort and protection.

Generally, mouth guards cover your upper teeth only, but in some instances (such as if you wear braces or another fixed dental appliance on your lower jaw), your dentist will make a mouth guard for the lower teeth as well. Your dentist can suggest the best mouth guard for you. An effective mouth guard should be comfortable, resist tears, be durable and easy to clean, and should not restrict your breathing or speech.

If you grind your teeth at night, a special mouth guard-type of dental appliance -- called a nocturnal bite plate or bite splint -- may be created to prevent tooth damage.

Oral Cancer Detection Devices

  1. Brush biopsy – “The OralCDx® BrushTest®is an easy, painless and definitive way for dentists to test the common small white and red oral spots that most people have in their mouth at one time or another. The BrushTest is used to determine if a common oral spot contains abnormal cells (known as dysplasia) that, if left alone for several years, may develop into oral cancer.”
  2. ViziLite Plus, MicroLux DL – A light technology that “in combination with a regular visual examination, provides a comprehensive oral screening procedure for patients at increased risk for oral cancer.”
  3. Velscope - “The ‘white light’ exam. The VELscope exam is actually the second step in a comprehensive oral cancer exam. The first step is a conventional ‘white light’ exam in which the dentist or hygienist looks for lesions in the oral cavity with the naked eye and uses palpation to feel for any bumps in the neck or face.”
  4. Trimira OCS 3000 – Trimira’s®, new Identafi® 3000 ultra uses the Identafi® Multi-Spectral Fluorescence and Reflectance technology to enhance visualization of mucosal abnormalities such as oral cancer or premalignant dysplasia that may not be apparent to the naked eye. But unlike other fluorescence technologies and dye systems, the Identafi® 3000 ultra is Multi-Spectral with three distinct color wavelengths, making it easier to distinguish lesion morphology and vasculature thereby reducing false positives.

The oral cancer detection devices mentioned above are only tools for screening and are not designed to diagnose oral cancer. They result false negatives if not used appropriately.  The ideal screening is a conventional visual oral tactile exam at the dental office first.

Visiting a pathologist in suspicious cases is highly recommended.  A second opinion can save you a lifetime. Do not wait and watch the cancer grow in your mouth.
